Record Release

If you would like a copy of your pet’s medical records sent to you or transferred to another veterinary hospital, please complete the Records Release Request Form below.

Once submitted, your request will be reviewed by the veterinarian before the records are released to the email address you specify. To ensure accuracy and completeness, please allow 3–5 business days for processing.

If you have an urgent need for records, we recommend contacting our team directly so we can determine if accommodations can be made.
