Becoming A Client

Please find below our new client forms for dog and cat patients and exotics. If your pet has any previous medical history from another veterinarian, clinic, or breeder, we also ask to have this history forwarded to us. It is preferred that we receive the completed New Client Paperwork and medical history before scheduling an appointment. This allows our experienced staff to review your pets’ individualized history to ensure we are appropriately scheduling for services they may need.

Completed New Client Paperwork and Medical History can be emailed directly to us at info@northsideanimalhospital.com.

If you do not have access to a printer, we welcome you to visit our office, and we can happily provide this paperwork in person and schedule a visit for your pet while you wait.
